Classical texts describe Sthaulya as accumulation of meda-dhatu (fat tissue) due to Kapha aggravation, weak Agni (digestive fire), and sedentary lifestyle. Charaka Samhita lists 8 Nindita Purusha (the obese being one of the 8 unhealthy types) and outlines specific risks: shortened lifespan, diabetes, joint disease, fatigue. The Kerala approach combines internal Lekhana herbs with external Udvartana (medicated powder massage) and graded Vyayama (exercise).
Kerala treatments
Udvartana (Kolakulathadi Choornam massage) is the signature Kerala Lekhana therapy — daily for 14–21 days, mobilises subcutaneous fat. Combined with Virechana Panchakarma to clear Kapha-Medas, dosha-typed diet, and progressive Yoga. Weight loss is gradual (2–4 kg/month) but sustained, with measurable improvement in lipid profile + insulin sensitivity.
