Categories of Visha
Sthavara visha (immovable) Plant and mineral toxins. Examples: vatsanabha (aconite), bhallataka (marking nut), shankha-puspi excess, certain mushrooms, copper sulphate.

Sushruta's 24 Vegas
Sushruta enumerates 24 progressive stages of visha vega (poison effect) after a snake bite. The first 7 are observable within minutes; later vegas progress over hours. Modern toxicology recognises similar pharmacokinetic-pharmacodynamic stages.
Vegas (selected): 1. Sharira-tashana (local burning) 2. Rakta dhatu involvement (discolouration spread) 3. Mamsa dhatu involvement 4. Meda dhatu involvement 5. Asthi-majja involvement 6. Manas vibhrama (mental disturbance) 7. Hridaya stambhana (cardiac arrest stage)
A patient at vega 1-3 has high survival; vega 6-7 is often fatal even with modern ASV.
Treatment Principles
Local management 1. **Arishta bandhana** — proximal tourniquet (loose, just venous flow blocked, not arterial) 2. **Pracchana** — incision over bite site (1-2 cm cross-cut) 3. **Rakta-mokshana** — bleeding to remove venom-laden blood 4. **Vamana** if recent oral ingestion 5. **Kashaya parisheka** — washing with specific kashaya (haridra, kushta) 6. **Lepa** — topical paste of agada drugs
Systemic management 1. **Specific antidote agadas** — Bilwadi gutika, Mritasanjeevani, Dushivishari, etc. 2. **Mantra chikitsa** — Kerala visha vaidyas use specific mantras alongside aushadha 3. **Snehapana** — small amount of ghrita as anti-toxin 4. **Anjana** — eye drops to reverse some neurotoxin effects 5. **Sirovirechana** with specific nasya for neurological involvement
Specific to species - **Cobra (Naga) bites**: neurotoxic; focus on respiratory support + sarpagandha + ksheerakakoli decoctions - **Russell's viper (Mandali)**: haemotoxic; focus on rakta-stambhana + manjishtha + lodhra - **Krait (Karkotaka)**: neurotoxic + cardiotoxic; aggressive systemic agada protocol
Classical Agada Formulations
Mritasanjeevani Gutika ~30 ingredients including hingu, jatamansi, brahmi, vacha, gomutra-shodhita ghrita. Used for severe toxic encephalopathy.
Bilwadi Gutika Bilva + lashuna + black salt + others. Used for snake-bite, scorpion sting, food poisoning, viral fevers (broad antidote).
Dushivishari Agada 26 ingredients. Specifically for chronic, low-grade, accumulated toxicity. Used in chronic skin disease, autoimmune conditions, drug overdose.
Maha-tikta Ghrita Bitter ghrita. Used in shighra (acute) and cumulative toxin elimination.
Modern Integration
In Kerala today, visha vaidyas often work alongside modern emergency medicine:
- **Anti-snake venom (ASV)** is the first-line for venomous snake bites (recommended by IMA, WHO)
- **Ayurveda agada** is used **adjunctively** for residual paresis, chronic effects, recovery
- **Pre-hospital triage** by visha vaidyas in remote Kerala villages — they decide if hospital transport is needed, give first-line agada en route
Outcomes data: a 2018 retrospective study from Kerala showed that snake bite victims given **combined Ayurveda + ASV** had: - Lower morbidity at 30 days vs ASV-only - Faster wound healing - Less neuropathic sequelae
