The Goal of Garbhini Paricharya
Charaka defines four goals (Sharira 8): 1. **Garbha-sthapana** — securing the pregnancy (preventing miscarriage) 2. **Garbha-poshana** — nourishing the foetus 3. **Garbha-rakshana** — protecting from harm (infections, exposures, malformations) 4. **Garbhini-rakshana** — protecting the mother
The framework runs from confirmation of pregnancy through delivery, with month-specific protocols.
Month-by-Month Protocol
Pratima Masa (1st month) - **Aahara**: cold, sweet, liquid foods. Milk frequently. Avoid: spicy, sour, salty, fermented. - **Vihara**: rest, avoid sudden movements, no excessive exertion. - **Aushadha**: shadangapaniya, ksheera with shatavari kalka. - **Goal**: stabilise the garbha (vulnerable to spontaneous abortion).
Dvitiya Masa (2nd month) - **Aahara**: continued sweet, cold liquid foods + small amount of meat-juice (mamsa rasa) in non-vegetarian context, otherwise enriched milk. - **Vihara**: continued rest, no marital relations. - **Aushadha**: ksheerapaaka with ashwagandha or jeevanti.
Tritiya Masa (3rd month) - **Aahara**: madhura ksheera + ghrita + cooked rice with milk (payasam-style). - **Vihara**: gentle walk introduced. - **Aushadha**: brimhana focus. - **Significance**: foetal sense organs (jnanendriya) begin to form.
Chaturtha Masa (4th month) - **Aahara**: navaneeta (butter), ksheera, mamsa rasa, vegetarians take cooked sashtika rice + ghee + sweet preparations. - **Vihara**: more movement allowed. - **Aushadha**: bala-ksheera-pachana. - **Significance**: foetal dhatus differentiate; hridaya formation.
Panchama Masa (5th month) - **Aahara**: ghrita + ksheera with sweet preparations. - **Vihara**: cradle environment, calm thoughts. - **Aushadha**: rasayana ghrita preparations. - **Significance**: mamsa dhatu formation; foetus starts movement felt by mother.
Shashta Masa (6th month) - **Aahara**: gokshura ksheera, ghrita with rasayana herbs. - **Vihara**: peaceful music, positive thoughts (foetal manas formation). - **Aushadha**: gokshura, draksha, madhura herbs. - **Significance**: bala (strength) + varna (complexion) develop.
Saptama Masa (7th month) - **Aahara**: ksheera + ghrita with shatavari, ashwagandha. - **Vihara**: avoid heavy bathing, alcohol, excessive sexual stimulation in environment. - **Aushadha**: special focus on rasayana for foetal majja (brain) formation. - **Significance**: foetus is fully formed; viable.
Ashtama Masa (8th month) - **Aahara**: ksheera with shali rice payasam, salted barley water in small amounts. - **Vihara**: rest, no straining, preparation for delivery. - **Aushadha**: anuvasana basti with bala-taila — softens birth canal. - **Significance**: foetus draws ojas from mother → mother feels weakness.
Navama Masa (9th month) - **Aahara**: light, easy-to-digest meals. - **Vihara**: prepare delivery space (sutika-griha); have midwife/doctor available. - **Aushadha**: yoni-pichu with bala-taila daily. Anuvasana basti for softening. - **Goal**: smooth labour and delivery.
Three Master Principles
1. Sthapana (stabilisation) First trimester focus. Use madhura, sheeta, mridu dravyas. Avoid panchakarma. Treat any complications with conservative, drug-light methods.
2. Vatashamana (vata pacification) Throughout pregnancy. Vata aggravation = miscarriage, premature labour, hypertensive disorders. Maintain regular meals, sleep, snigdha-ushna diet, avoid travel last trimester.
3. Ojas-vardhana (ojas augmentation) Mother + foetus draw ojas from the mother's dhatu reserves. Use ojas-promoting dravyas: ksheera, ghrita, ashwagandha, shatavari, draksha, madhu. Especially critical 6th-9th months.
Contraindications During Pregnancy
- **Strict shodhana** (vamana, virechana, basti) — generally contraindicated except teekshna anuvasana basti in 8-9th month.
- **Raktamokshana** — contraindicated.
- **Strong heating herbs** — pippali high doses, guggulu, bhallataka, asparagus alone in some cases.
- **Excessive travel** especially in 1st and 3rd trimester.
- **Strenuous exercise**, marital relations after first trimester.
Modern Correlation
Many garbhini paricharya principles align with modern obstetric guidance:
- Frequent small meals = recommended for nausea, hypoglycaemia prevention
- Milk + ghee = calcium + DHA-equivalent for foetal brain
- Shatavari (Asparagus racemosus) = traditional galactogue; some modern evidence
- Avoid heating spices = mirror of modern caution with herbal supplements
- Rest in 3rd trimester = aligns with WHO recommendation
Kerala's Extended Tradition
After delivery, **Sutika paricharya** continues for 45-90 days: - Daily abhyanga with kshira-bala taila - Specific food preparations (kanji with jeera/methi, harishina/jaggery) - Pippali ksheerapaaka for lactation - 41-day mother-baby seclusion period — protects newborn from infection
This is one of Kerala's distinctive contributions and is regaining popularity globally.
